How AI describes 2-1-1 / PA Navigate

PA Navigate is a statewide network that connects Pennsylvanians with local health and social care services, allowing users to find resources such as food, housing, utilities, and transportation by entering their ZIP code. It operates through four state-certified Health Information Organizations and the Pennsylvania Patient & Provider Network to streamline referrals and share information about social needs and referral status among providers and payers. Funded by the Pennsylvania Department of Human Services, the project also provides crisis resources (988 Suicide and Crisis Lifeline) and invites organizations to participate in its network.
