What AI says about Outreach Grid

Outreach Grid is a software platform from Appledore that helps communities scale best practices to combat homelessness by coordinating outreach, shelter, housing, and case management across multiple agencies on one unified system. It enables tracking of vulnerable individuals, bed inventory management, and the creation of tailored service plans from first contact to housing, supporting progress over time. The company provides assessments, whitepapers, demos, and pricing information to help communities adopt and scale the system, including tools like the Community Technical Readiness Assessment to evaluate local readiness.
