Kidney Choices in AI answers

IPRO offers mobile and web-based CKD screening and patient-centered care apps (Clinician CKD Screening and Kidney Choices) designed to identify CKD early, help manage CKD, delay progression, and provide information on kidney replacement. The Clinician CKD Screening app provides clinical decision support for care management and appropriate labs; the Kidney Choices app supports patient self-management and shared decision making with two tracks: CKD on Track (tracking Medicare data and labs/visits with due-date notifications) and Kidney Treatment Choices (exploring ESRD treatment options and preparing questions for providers). These apps can be used alone or embedded in IPRO's Chronic Kidney Disease Awareness & Prevention Program to raise awareness, educate patients, and support provider-patient communication, including a CKD Conversation Starter for clinicians.
